Provider Portal
Create a Provider Portal account and gain access to valuable online tools and features including member eligibility and claim status, deductible and out-of-pocket information, ID card images, plan information, and various self-service forms and reports.

Need quick access? FastTrack lets you check claim and eligibility status instantly—no login required. For more robust tools, take a moment to register for a full account.
Documents
Review and download useful documents including various claims and formulary drug forms, retail, and specialty pharmacy requests and important tax forms.
Electronic Claims
Providers: Submit your claims directly to Allied through a clearinghouse and get paid faster.
Allied has two payer IDs:
- For Allied Benefit Systems, use 37308.
- For Allstate Benefits use 75068.
Please refer to the Member ID card for the correct payer ID. Always use the payer ID shown on the ID card.
